Home Health Registered Nurse (RN) | Field Visits
About The Position
As an RN Visit Nurse, you'll provide 1:1 skilled nursing care that allows patients to recover safely, remain independent, and stay where they want to be most: at home. Requirements
- Active Minnesota Registered Nurse (RN) license
- Experience in home health, hospital, rehabilitation, skilled nursing, or other clinical settings
- Strong assessment and critical thinking skills
- Excellent communication and relationship-building abilities
- Ability to work independently while managing daily visits
- Reliable transportation and willingness to travel throughout the St. Paul territory
Responsibilities
- Provide skilled home health nursing visits throughout the St. Paul service area
- Perform comprehensive patient assessments
- Deliver wound care, medication management, injections, and other skilled nursing services
- Educate patients and caregivers on diagnoses, medications, and treatment plans
- Monitor patient progress and communicate changes with physicians
- Collaborate with therapists, social workers, and interdisciplinary care teams
- Complete timely documentation in Homecare Homebase (HCHB)
- Promote positive patient outcomes while helping reduce unnecessary hospitalizations
