As an RN Visit Nurse, you'll provide 1:1 skilled nursing care that allows patients to recover safely, remain independent, and stay where they want to be most: at home. Requirements

  • Active Minnesota Registered Nurse (RN) license
  • Experience in home health, hospital, rehabilitation, skilled nursing, or other clinical settings
  • Strong assessment and critical thinking skills
  • Excellent communication and relationship-building abilities
  • Ability to work independently while managing daily visits
  • Reliable transportation and willingness to travel throughout the St. Paul territory

Responsibilities

  • Provide skilled home health nursing visits throughout the St. Paul service area
  • Perform comprehensive patient assessments
  • Deliver wound care, medication management, injections, and other skilled nursing services
  • Educate patients and caregivers on diagnoses, medications, and treatment plans
  • Monitor patient progress and communicate changes with physicians
  • Collaborate with therapists, social workers, and interdisciplinary care teams
  • Complete timely documentation in Homecare Homebase (HCHB)
  • Promote positive patient outcomes while helping reduce unnecessary hospitalizations
