About The Position
Alpha Home Health & Hospice is hiring a full-time Home Health Registered Nurse (RN) for their King County area Home Health division. This role focuses on providing one-on-one patient care for complex medical needs in the home setting. The position involves conducting initial and ongoing patient assessments, developing and updating individualized care plans, administering medications, treatments, and injections, performing wound care, monitoring vital signs, educating patients and caregivers, coordinating care with other healthcare professionals, documenting all visits in the EMR, ensuring compliance with regulations, participating in case conferences, and providing emotional support to promote patient independence.
Requirements
- Washington RN licensure
- Current BLS certification
- Valid driver’s license and insured vehicle

- Home Health/Hospice agency experience preferred
Responsibilities
- Conduct initial and ongoing patient assessments in the home setting
- Develop and update individualized care plans in collaboration with the care team
- Administer medications, treatments, and injections as prescribed
- Perform wound care and dressing changes and other complex care
- Monitor vital signs and symptoms, reporting changes to the physician
- Educate patients and caregivers on disease processes, medications, and self-care
- Coordinate care with physicians, therapists, and other healthcare professionals
- Document all visits and care provided in the EMR
- Ensure compliance with agency policies, Medicare/Medicaid, and other regulations
- Participate in case conferences and interdisciplinary team meetings
- Provide emotional support and promote patient independence
