Requirements

Current Arizona Registered Nurse (RN) License or Compact RN License valid in Arizona
Graduate of an accredited nursing program
Current CPR Certification
Valid driver's license
Reliable transportation and current auto insurance
Strong clinical assessment, communication, and organizational skills
Ability to work independently in a home-based clinical environment while collaborating with an interdisciplinary team
Currently licensed as a Registered Nurse to practice in the State of Arizona (or) Current multi-state license from another compact state utilizing their valid compact state license for up to 30 days until an Arizona permanent license is issued.

Responsibilities

Manage an assigned caseload within the Southwest Phoenix Valley
Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
Perform comprehensive nursing assessments and develop individualized plans of care
Provide skilled nursing care based on each patient's clinical needs
Perform wound care and monitor healing and treatment effectiveness
Provide medication management, reconciliation, and patient education
Monitor changes in patient condition and communicate promptly with physicians and the interdisciplinary care team
Educate patients and caregivers on disease management, medications, safety, and prevention of complications
Coordinate care with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, Home Health Aides, physicians, and other healthcare professionals
Complete timely and accurate documentation in accordance with Medicare, regulatory, and agency requirements
Help prevent avoidable hospitalizations and emergency department visits
Support patients in achieving greater independence, safety, and quality of life at home
