About The Position

Concierge Home Care is hiring an experienced Home Health Registered Nurse (RN) Preceptor in the Southwest FL area. This is a unique leadership opportunity for an RN who enjoys mentoring clinicians, supporting clinical excellence, and maintaining a manageable patient caseload. If you're ready to step into a leadership-focused role while keeping flexibility and autonomy, this opportunity is built for you. This role offers leadership with flexibility, allowing you to serve as a clinical mentor with a reduced patient caseload. The schedule is Monday-Friday with no nights, weekends, on-call, or 12-hour shifts, and provides autonomy to manage your daily schedule and workflow. You will also have regional travel supporting multiple branches. You will make a meaningful impact by training and developing new and existing clinicians, improving patient outcomes through hands-on coaching, and serving as a trusted clinical resource across your region. The company utilizes technology that supports you, including AI-assisted charting to reduce documentation time and an efficient EMR system (WellSky/Kinnser), allowing you to spend more time coaching and less time charting.

Requirements

  • Active, unencumbered Florida RN license (required)
  • Minimum 2 years of home health experience (required)
  • Strong knowledge of Medicare OASIS documentation (required)
  • Valid driver’s license, reliable transportation, and auto insurance
  • Ability to travel within the assigned territory

Nice To Haves

  • Experience with WellSky/Kinnser EMR (preferred)

Responsibilities

  • Conduct co-visits to train and support field clinicians
  • Lead clinical onboarding and orientation sessions
  • Complete competency evaluations and skill validations
  • Provide ongoing mentorship and clinical guidance
  • Perform patient assessments, OASIS documentation, and care planning
  • Deliver skilled nursing care, including: Chronic and acute disease management, Medication management, Wound care and post-surgical care, Pain management
  • Collaborate with physicians and interdisciplinary teams
  • Support quality assurance and compliance initiatives
  • Travel within Sarasota County and surrounding areas as needed
