The physical therapist is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement. This role involves assessing and evaluating patients' therapeutic, rehabilitative, and functional status, developing a total plan of care, and directing physical therapy treatment. The therapist will also instruct patients, families, and caregivers, identify needs for other home health services, and participate in discharge planning. Additionally, the role includes providing consultation, in-service education, supervising assistants and aides, and maintaining CPR certification. A valid driver's license and insured automobile are required.
