About The Position

Therapy Partner Solutions (TPS), a therapist-owned and operated company since 2016, is seeking a Physical Therapist for home health coverage in Northeast Volusia County (Port Orange/Daytona). As the provider of home health therapy services for Accent Care Home Health, TPS is part of a strong, multidisciplinary team dedicated to providing exceptional therapy services. They understand the unique challenges clinicians face and are committed to supporting their staff through mentorship, advanced training, and professional development opportunities. Requirements

• Graduate (current or pending) from an accredited physical therapy program.
• Active or pending state physical therapist license.
• Comfort working independently in the home setting.

Responsibilities

• Provide exceptional therapy services in a home health setting.
• Work with a diverse and engaging caseload.
• Collaborate with a multidisciplinary team, including physicians and other healthcare professionals, for coordinated, patient-centered care.
• Manage caseload with thoughtful scheduling and defined territories.
• Minimize drive time between visits.
