About The Position
Hillcrest Health & Living is seeking a compassionate and motivated Physical Therapist (PT) to join our Home Care team in Omaha, Nebraska. This role provides the opportunity to deliver meaningful, one-on-one care that helps individuals regain strength, mobility, and confidence—all in the comfort of their own homes. Part-time or PRN available!
Requirements
- Licensed Physical Therapist (PT) in the State of Nebraska
- Valid driver’s license and reliable transportation
- Excellent communication and documentation skills
- Commitment to integrity, compassion, respect, teamwork, and service
Responsibilities
- Evaluate patients and create personalized treatment plans to improve function, mobility, and overall well-being.
- Provide physical therapy services in client homes while ensuring the highest quality of care.
- Collaborate with nurses, occupational therapists, and other members of the interdisciplinary team.
- Accurately document care and communicate progress to ensure seamless coordination.
- Educate patients and families on exercise, safety, and home programs to support lasting results.
