About The Position

The Physical Therapist plans and provides physical therapy services to clients in their homes in accordance with each Plan of Care and company policies and procedures. The role requires specialized knowledge, judgment, and skill grounded in psychological, biological, and social service principles. The Physical Therapist reports directly to the Clinical Supervisor. This is a full-time or part-time position with a flexible schedule during office hours, Monday through Friday. The role requires travel to clients’ homes to provide one-to-one care, which may take one to three hours per client. The position reports to the St. Cloud, Minnesota office and serves Little Falls, Pierz, and surrounding cities.

Requirements

Graduate of an accredited Physical Therapy program.
Currently licensed as a Physical Therapist in Minnesota; license must not be revoked or suspended and must be free of limitations or restrictions.
At least two years of experience in a health care setting or equivalent experience.
Strong interpersonal communication and teaching skills.
Strong organizational skills and ability to work independently.
Excellent written and oral communication skills.
Access to a dependable vehicle or public transportation for travel to multiple company business stops per day.
Valid driver’s license and proof of car insurance when using a personal vehicle for company business.
Proof of a negative Mantoux test or documentation of a negative chest X-ray.
U.S. citizenship or evidence of a valid Alien Work Permit.
Disclosure of convictions and criminal history records related to crimes involving the provision of health services; candidates convicted of such crimes will not be hired.
Pass initial and ongoing background studies and screenings, including those of the Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.

Nice To Haves

Home care experience preferred.
Current CPR certification is highly recommended.

Responsibilities

Administer physical therapy evaluations and participate in developing Plans of Care.
Provide prescribed physical therapy treatments, procedures, evaluations, and diagnostic tests under the direction of clients’ physicians.
Translate exercises into functional activities or activities of daily living.
Instruct clients in the use and care of assistive devices.
Develop plans to modify equipment, appliances, and the physical surroundings in clients’ homes as needed.
Prepare clinical and progress notes and report clients’ responses to treatment or changes in condition to physicians and Case Managers.
Maintain appropriate records, including visit frequency and clients’ responses.
Instruct and supervise PTAs and HHAs when appropriate.
Provide in-service education programs for company staff as requested.
Participate in Case Conferences as indicated.
Provide families with information, support, and encouragement; instruct family members on clients’ home physical therapy programs.
Help individuals and families accept and adjust positively to physical, mental, and social limitations.
Participate in developing community resources to meet clients’ needs.
Maintain positive and effective communication with staff and others and participate in the team concept.
Maintain confidentiality of information pertaining to clients, families, and employees.
Maintain a safe client environment and report suspected abuse, neglect, or financial exploitation of vulnerable clients to the office.
Perform other related duties and responsibilities as assigned.
