About The Position

The Home Health Physical Therapist provides in-home care to patients in the Bandera, Texas area. The role evaluates patients’ physical abilities and functional limitations, develops and implements individualized treatment plans, and delivers therapeutic interventions to restore function, improve mobility, and alleviate pain. The therapist educates patients and caregivers, monitors progress, adjusts treatment plans, conducts home safety assessments, and collaborates with an interdisciplinary care team.

The position uses the Roger app, a smart, voice-enabled, HIPAA-compliant documentation platform, on most visits to help streamline documentation while meeting OASIS and compliance requirements. The role offers schedule flexibility and independence while focusing on compassionate patient care.

Requirements

• Active Physical Therapist license in the state of Texas.
• Professional liability insurance coverage specific to the Texas Physical Therapy license.
• Graduate degree or higher in Physical Therapy from an accredited program.
• Ability to adapt care approaches to a variety of patient personalities and needs.
• Up-to-date knowledge of evidence-based treatment techniques and best practices.
• Excellent communication skills and a friendly, professional demeanor when educating and interacting with patients.

• Experience in home health or rehabilitative settings is preferred.

Responsibilities

• Evaluate patients’ physical abilities and functional limitations in a home health setting.
• Develop and implement individualized treatment plans to restore function, improve mobility, and alleviate pain.
• Provide therapeutic interventions, including exercise, gait training, manual therapy, and balance retraining.
• Educate patients and caregivers on home exercise programs, safety techniques, and adaptive strategies.
• Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
• Document evaluations, treatment sessions, and progress notes using digital platforms.
• Collaborate with interdisciplinary team members, including physicians, nurses, and occupational therapists.
• Conduct home safety assessments and recommend measures to prevent falls or re-injury.
• Deliver compassionate care while empowering patients to achieve greater independence.
