About The Position

Concierge Home Care is seeking a Home Health Physical Therapist (PT) to join their team in the Sarasota County area. This role offers more control over your schedule, the opportunity for meaningful patient relationships, and the potential to increase income based on productivity. The position involves 1:1 patient care in the home, with territories designed to minimize drive time through clustered scheduling. The company utilizes AI-assisted charting and a user-friendly EMR (WellSky/Kinnser) to reduce documentation time, allowing therapists to focus more on patient care. They also provide structured onboarding, mentorship, preceptorship, ongoing training, free CEUs, and career advancement opportunities.

Requirements

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
• Successful completion of an AHCA Level 2 background screening

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
