About The Position

As a therapist at CenterWell Home Health, you'll help patients regain strength, mobility and independence—all from the comfort of their homes. You will report to the Clinical Manager.

Requirements

• Current and unrestricted Physical Therapy license in the state of North Carolina
• A valid driver's license, auto insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Confer with physician and clinical team members to obtain patient information and help develop, implement and revise the therapy treatment program.
• Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function
• Submit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staff
• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions and care plan that reflects current plan of care as related to therapy
• Oversee and evaluate Physical Therapy Assistant and Home Health Aide staff performance, including monitoring the appropriate completion of documentation
• Report patient progress to the patient's physician, Clinical Manager, staff, patient and family
