About The Position

Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, and a consistent Top Workplace. This role is for a Weekend Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Bloomfield Hills area. The position involves delivering skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team. Responsibilities include performing comprehensive patient evaluations, functional mobility assessments, and developing individualized therapy plans to improve strength, balance, and independence. The role also involves educating and empowering patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies, ensuring thorough and compliant EMR documentation, and working independently while traveling to patient homes within the assigned territory.

Requirements

Active Physical Therapist, PT license in the state you work
One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
Work independently, but never alone
Travel to patient homes within assigned territory
