We are hiring a Weekend Physical Therapist (PT) for a Home Health position providing skilled care to patients in their homes in the Bloomfield Hills area. This role offers autonomy, meaningful patient relationships, and career growth.
Requirements
- Active Physical Therapist, PT license in the state you work.
- One year of experience as a Physical Therapist.
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities
- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team.
- Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence.
- Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies.
- Ensure thorough, compliant EMR documentation.
- Travel to patient homes within assigned territory.
