About The Position
Concierge Home Care is seeking a Home Health Physical Therapist (PT) who desires more control over their schedule, meaningful patient relationships, and the ability to increase their income based on productivity. This role offers flexibility, autonomy, and the opportunity to provide 1:1 patient care in the home within the Mt. Dora/Eustis/Tavares, FL / Lake County area. The position utilizes technology to reduce documentation time and offers structured onboarding, mentorship, and ongoing training.
Requirements
- Active, unencumbered Florida Physical Therapist license
- Valid driver’s license, reliable transportation, and auto insurance
- Employment is contingent upon successful completion of an AHCA Level 2 background screening.
Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and caregivers on safety and recovery
- Collaborate with physicians and interdisciplinary teams
- Supervise Physical Therapist Assistants (PTAs)
- Complete documentation using streamlined technology
