About The Position

Windsor Home Health, a division of PREMIER THERAPY, is seeking a Physical Therapist to provide care to patients in their homes and help them remain comfortable and independent. The role focuses on evaluating patients’ functional abilities, developing individualized treatment plans, providing therapy services, and supporting patients and caregivers with education and safety strategies. The therapist collaborates with an interdisciplinary team, documents care, monitors progress, and updates plans and goals as needed. Home health experience is preferred but not required. A PRN option is also mentioned.

Requirements

Graduate of an accredited Physical Therapy program.
Current Physical Therapist license in the state of Ohio.
Strong communication and organizational skills.
Valid driver’s license and reliable transportation.

Responsibilities

Evaluate patients’ functional abilities and develop individualized treatment plans.
Provide therapy services in the home setting.
Educate patients and caregivers on adaptive techniques and safety strategies.
Collaborate with physicians, nurses, therapists, and interdisciplinary team members.
Complete timely and accurate documentation.
Monitor patient progress and adjust treatment plans as needed.
Participate in interdisciplinary discussions and reviews of scheduled patients and treatment.
Evaluate new admissions and review potential discharges.
Provide staff training and communicate patient progress.
Develop or manage programming involving wheelchairs, splints, and adaptive equipment.
Communicate with patients’ families about progress, updated goals, adaptive equipment, and changes in patient status.
