About The Position

Advantage Home Health is seeking a compassionate and dedicated Physical Therapist to join our growing Home Health team. This PRN position offers the flexibility of managing your own schedule while making a real difference in patients' lives by providing one-on-one care in their homes. The role focuses on helping individuals regain strength, mobility, and independence in a comfortable environment.

Requirements

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in Pennsylvania.
• Current CPR certification (or ability to obtain upon hire).
• Valid driver's license, reliable transportation, and proof of auto insurance.
• Strong communication, organizational, and documentation skills.

Nice To Haves

• Home health experience preferred, but not required.

Responsibilities

• Provide skilled physical therapy services in accordance with physician orders and individualized plans of care.
• Evaluate patients and develop personalized treatment programs to improve function and quality of life.
• Assess mobility, strength, balance, and safety needs.
• Document patient evaluations, treatments, and progress in a timely and accurate manner.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes.
• Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals.
• Supervise Physical Therapist Assistants as appropriate.
