About The Position

Advantage Home Health is seeking a compassionate and dedicated Physical Therapist to join our growing Home Health team. This PRN position offers the flexibility of a PRN schedule while making a real difference in patients' lives. As a Home Health Physical Therapist, you'll enjoy the autonomy of managing your schedule, providing one-on-one care in patients' homes, and helping individuals regain strength, mobility, and independence where they are most comfortable.

Requirements

Graduate of an accredited Physical Therapy program.
Current Physical Therapist license in Pennsylvania.
Current CPR certification (or ability to obtain upon hire).
Valid driver's license, reliable transportation, and proof of auto insurance.
Strong communication, organizational, and documentation skills.

Nice To Haves

Home health experience preferred, but not required.

Responsibilities

Provide skilled physical therapy services in accordance with physician orders and individualized plans of care.
Evaluate patients and develop personalized treatment programs to improve function and quality of life.
Assess mobility, strength, balance, and safety needs.
Document patient evaluations, treatments, and progress in a timely and accurate manner.
Collaborate with physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes.
Educate patients and caregivers on exercises, safety techniques, and ongoing therapy goals.
Supervise Physical Therapist Assistants as appropriate.
