About The Position

Billet Health is looking for a compassionate and dedicated Physical Therapist (PT) to join our Home Health team in the West Valley. This per diem opportunity offers flexibility and autonomy while allowing you to provide one-on-one care that truly makes a difference. As a Home Health Physical Therapist, you will evaluate and treat patients in their homes, helping them regain mobility, improve function, and maintain independence.

Requirements

• Graduate of an accredited Physical Therapy program
• Current Arizona Physical Therapist license
• Valid driver's license and reliable transportation
• Current auto insurance
• CPR or BLS certification
• Current TB documentation (skin test within one year or chest X-ray within five years)

Nice To Haves

• Home Health experience preferred

Responsibilities

• Perform comprehensive patient evaluations and assessments
• Develop individualized treatment plans based on physician orders
• Provide skilled physical therapy services in patients' homes
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on exercises, mobility, and safety
• Collaborate with physicians and interdisciplinary team members
• Complete accurate and timely documentation
