About The Position

As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence, from the comfort of their homes. By delivering personalized care that focuses on functional improvement, you'll empower individuals to overcome limitations and enjoy a better quality of life. As a Home Health Physical Therapist, you will plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain the highest level of physical function. You will assess and document patients' physical strengths to support physician evaluation and guide treatment goals. You will develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team. You will provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation. You will document patient observations, interventions, and evaluations promptly and thoroughly. You will report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries. Requirements

Degree from an accredited Physical Therapy Program (approved by the APTA)
Current and unrestricted Physical Therapy license
Current CPR certification
Experience collaborating with a team of healthcare professionals
