About The Position

As a therapist at CenterWell Home Health, the Home Health Physical Therapist Assistant helps patients regain strength, mobility, and independence in their homes. The position delivers personalized care focused on functional improvement and administers the treatment plan developed by a Physical Therapist. The role reports to the Clinical Manager and includes timely clinical documentation, participation in educational meetings and department in-services, reporting equipment or supply issues and changes involving staff or clients, and applying infection-control practices and universal precautions. This is a patient-facing role subject to Humana's tuberculosis screening program and driver safety requirements.

Requirements

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population.
Fluent in reading, writing, and speaking English.

Responsibilities

Administer the Physical Therapist-developed treatment plan to patients in their homes.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, heat, light, cold, water, and electrical modalities.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director of equipment failures, lack of supplies, or changes involving staff or clients.
Apply infection-control concepts and universal precautions while performing patient care activities.
