About The Position

As a therapist at CenterWell Home Health, the Home Health Physical Therapist Assistant reports to the Clinical Manager and helps patients regain strength, mobility, and independence in their homes. The role delivers personalized care focused on functional improvement and quality of life. The assistant implements treatment plans developed by a Physical Therapist, documents patient care, participates in education and coordination activities, and follows infection-control and universal-precaution practices.

Requirements

Current state license to practice Physical Therapy.
Valid driver's license, auto insurance, and reliable transportation.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population.
Fluent English reading, writing, and speaking skills.
One year of experience as a Physical Therapy Assistant.

One year of experience in Home Health.

Responsibilities

Administer the physical therapy treatment plan to patients in their homes as developed by the Physical Therapist.
Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities to patients.
Document client records and submit clinical documentation to the office on time each week.
Attend educational meetings and coordinate department in-services.
Notify the Physical Therapist or Branch Director of equipment failures, lack of supplies, or changes in staff or clients.
Apply concepts of infection control and universal precautions while performing patient care activities.
