About The Position

Haven Home Health & Hospice is seeking a Physical Therapist Assistant to provide skilled physical therapy services to clients in their homes. The PTA works alongside a collaborative team of therapists and clinicians, helping clients remain safe, independent, and active while building relationships with clients and families and supporting the goals in each individualized plan of care.

Requirements

• Current Missouri PTA license in good standing.
• At least one year of PTA experience within the last three years.
• Strong communication and organizational skills.
• Ability to build meaningful relationships with clients, families, and fellow team members.
• Strong clinical judgment and ability to work independently.
• Comfort with technology and electronic documentation.
• Dependable transportation and a current driver’s license.
• Flexibility to work weekends and holidays as needed.

Responsibilities

• Provide skilled physical therapy services under the supervision of a Physical Therapist.
• Implement individualized plans of care based on physician orders and client needs.
• Monitor client progress and communicate changes with the supervising Physical Therapist and care team.
• Provide preventive and rehabilitative therapy interventions.
• Educate clients and families on self-management, safety, and physical therapy needs.
• Maintain accurate and timely clinical documentation.
• Collaborate with other Haven clinicians to provide coordinated, client-centered care.
• Participate in department meetings, in-services, clinical outcomes, and performance improvement initiatives.
• Uphold Haven policies, professional standards, and applicable regulatory requirements.
