Home Health Physical Therapist Assistant - PRN
About The Position
As a therapist at CenterWell Home Health, this role reports to the Clinical Manager and helps patients regain strength, mobility, and independence in their homes. The position delivers personalized care focused on functional improvement and quality of life. The Home Health Physical Therapist Assistant administers treatment plans developed by a Physical Therapist, provides therapeutic exercises and modalities, documents care, participates in education and in-services, reports equipment or patient-related changes, and follows infection-control precautions. The role is patient-facing and requires TB screening. It also involves driving and requires a valid state driver’s license and personal vehicle liability insurance that meets the applicable minimum limits.
Requirements
- Current state license to practice Physical Therapy.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
- Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population.
- Fluent in reading, writing, and speaking English.
- Must complete TB screening if selected for the patient-facing role.
- Must maintain personal vehicle liability insurance at applicable state minimum limits or the stated alternative limits, whichever is higher.

Responsibilities
- Administer the physical therapy treatment plan developed by the Physical Therapist to patients in their homes.
- Provide non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities.
- Document client records and submit clinical documentation to the office on time each week.
- Attend educational meetings and coordinate department in-services.
- Notify the Physical Therapist or Branch Director of equipment failures, lack of supplies, or changes involving staff or clients.
- Apply infection-control concepts and universal precautions while performing patient care activities.
