About The Position

CenterWell Home Health is seeking a Physical Therapist Assistant to help patients regain strength, mobility, and independence in their homes. Reporting to the Clinical Manager, the assistant delivers personalized care focused on functional improvement and administers the treatment plan developed by a Physical Therapist. The position is patient-facing and requires participation in Humana’s TB screening program, as well as compliance with its driver safety program. Florida-based applicants are subject to Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse.

Requirements

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of physical, emotional, social, and biological changes in the geriatric population.
• Read, write, and speak fluent English.

Nice To Haves

• One year of experience as a Physical Therapy Assistant.

Responsibilities

• Administer the Physical Therapist-developed treatment plan to patients in their homes.
• Provide non-complex active and passive manual therapeutic exercises, therapeutic massage, traction, and heat, light, cold, water, and electrical modalities.
• Document client records and submit clinical documentation to the office on time each week.
• Attend educational meetings and coordinate department in-services.
• Notify the Physical Therapist or Branch Director of equipment failures, supply shortages, or changes involving staff or clients.
• Apply infection-control concepts and universal precautions while performing patient care.
