Home Health Occupational Therapist
About The Position
BAYADA Home Health Care is seeking an experienced Occupational Therapist for a part-time position providing home health visits to adult and geriatric patients in their homes in Dartmouth, Westport, Fall River, and surrounding towns. Prior home health experience is not required. The Occupational Therapist is an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing so clients can receive the care needed to remain at home. The position involves evaluating patients’ functional status and developing individualized intervention plans, assessing home environments and equipment needs, coordinating care plans with nursing as appropriate, educating patients and caregivers, and documenting services using a touch pad tablet. The advertised base pay is $62.00–$67.00 per point, depending on qualifications.
Requirements
- Current Massachusetts Occupational Therapist license.
- At least one year of recent work experience as an Occupational Therapist.
Responsibilities
- Make home visits to clients in designated geographic territories.
- Perform thorough evaluations of patients’ functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
- Evaluate home environments for hazards or barriers to more independent living.
- Identify equipment needs and environmental adaptations to enhance client outcomes.
- Continually assess and revise occupational therapy care plans and participate with nursing in multidisciplinary care planning as appropriate.
- Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities that promote health, safety, and independent living.
- Accurately document observations, interventions, evaluations, patient care management, and services provided using a touch pad tablet.
