About The Position

BAYADA Home Health Care is hiring an experienced Occupational Therapist for a part-time position making home health visits in Uxbridge, Douglas, Webster, Oxford, and surrounding towns. The therapist will provide rehabilitative care to adult and geriatric patients in their homes as part of a multidisciplinary health care team, helping clients receive care at home. Prior home health experience is not required. The role includes evaluating patients’ functional status and performance, creating and revising individualized intervention and care plans, assessing home environments, identifying equipment and adaptation needs, educating patients and their representatives, and documenting care using a touch pad tablet. Compensation is $62.00–$68.00 per point, depending on qualifications.

Requirements

Current Massachusetts Occupational Therapist license.
At least one year of recent work experience as an Occupational Therapist.

Responsibilities

Make home visits to clients in designated geographic territories.
Evaluate patients’ functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
Evaluate home environments for hazards or barriers to independent living.
Identify equipment needs and environmental adaptations to improve client outcomes.
Continually assess and revise occupational therapy care plans and participate with nursing in multidisciplinary care planning, as appropriate.
Educate and instruct patients, family members, or other patient representatives in rehabilitative care and activities that promote health, safety, and independent living.
Document observations, interventions, evaluations, and services provided using a touch pad tablet.
