About The Position

The Home Health Occupational Therapist (OT) evaluates and implements plans of care to improve, restore, and enhance patients’ physical functional performance and promote injury prevention. The role also performs documentation and quality improvement activities in accordance with departmental policies and procedures. The position is based at Michigan City Coolspring Health Center in Michigan City, Indiana, and requires travel up to 75%.

Requirements

• Bachelor's degree in Occupational Therapy.
• One year of occupational therapy experience.
• Occupational Therapist state licensing board credential or license.
• American Heart Association Cardiopulmonary Resuscitation (CPR) certification.
• Driver's license.
• Auto insurance.

Nice To Haves

• One year of home health experience.

Responsibilities

• Perform comprehensive occupational therapy evaluations, treatment, and related follow-up services, including discharge planning.
• Document patient assessments and pertinent information in a timely manner.
• Develop treatment plans with measurable goals based on patient assessments.
• Provide occupational therapy, including selecting and using adaptive equipment.
• Collaborate with patients, healthcare personnel, and outside agencies to coordinate care.
• Perform documentation and quality improvement activities in accordance with departmental policies and procedures.
