Requirements

Graduate of an accredited school of nursing.
2+ years of recent acute care experience in an institutional setting.
1 year of Home Health experience required.
Valid driver’s license with no restrictions.
Auto insurance meeting state’s insurance coverage minimums.
Current licensure in State.
CPR certification.
Excellent observation, verbal and written communication skills.
Problem-solving skills.
Basic math skills.
Nursing skills per competency checklist.
Demonstrates an enthusiasm for the nursing and therapy profession and projects a positive attitude.
Conveys a commitment to personal and professional growth.

Nice To Haves

Bachelor’s degree preferred.
3+ years of Home Health experience preferred.
Supervisory experience not required but preferred.

Responsibilities

On-site supervision and assist with management of Nursing services for each branch.
Report directly to the Branch Director and partner with the clinical and quality team to support broader initiatives.
Indirectly supervise RN Field Staff, with overall direction from the Branch Director and support from Clinical, Quality, and Education teams.
Serve as a hybrid per-visit clinical role with reduced productivity expectations to allow for administrative time.
Meet with Branch Director weekly to review and plan agenda and duties, including interviewing, hiring, onboarding, on-site skills assessments, and supervisory visits.
Train LVN RN (if applicable) to perform basic in-services for the Business Development team.
Assist Branch Director and Rehab Management with staffing, including weekend and holiday Rehab coverage schedules.
Act as spokesperson during Care Team Conference to share updates, align on key metrics, discuss trends, or regulatory issues.
Oversee and run monthly nursing meetings for their branch.
Assist in regulatory oversight of all quality initiatives.
Assist in developing action plans (POC) post site survey credentialing and complaint visits.
Expand knowledge through sharing expertise and promoting learning by connecting theory with practice.
Incorporate essential elements of critical thinking and development of clinical practice.

Demonstrate collaboration by consistently integrating team-based communication strategies.
Clearly define relationships and identify expectations at the beginning of orientation.
Assist in identifying learning needs and develop an orientation schedule with various learning opportunities.

Include new hires in the development of learning objectives, attainable outcomes, and goals.
Apply effective communication strategies.
Select appropriate conflict management strategies when coaching for performance.
Give and receive performance feedback throughout the orientation process.
Validate clinical skills per the organization’s standards.
Provide objective evaluations of new hire’s clinical performance.
Complete all required documentation for orientation.
Assist new hires in integrating socially and professionally as a member of the interdisciplinary team.
Model professional verbal and non-verbal behaviors.
Encourage independence and self-confidence through the development of coping skills.
Maintain confidentiality of the evaluation and progress of the new hire.
Guide new employees in the orientation process, teaching roles, responsibilities, standards of behavior, rules, and culture.
Assist in completion of orientation checklists and competency validation.
Oversee the learning experience for students.
Mandatory attendance on new hire update calls.
Assist existing employees with roles and responsibilities, education on new and existing processes and programs.
Assist with annual skills competencies.
Be available for clinical conferencing and consulting from team members.
Attend mandatory quarterly team meetings with the education department and fellow preceptors.
