Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. This specific opportunity is for a kind and fun 18-year-old woman with a rare medical condition who requires trach and vent care. The home is non-smoking and has two small dogs. The role involves providing skilled nursing care, monitoring patients, assisting with daily activities, and communicating with families and care teams.
Requirements
- Active RN or LPN license in the state of Michigan
- Strong communication and critical thinking skills
Nice To Haves
- Home care or private duty experience preferred
- Trach/vent experience a plus (training provided)
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams
- Maintain safe, high-quality, patient-centered care
