Overnights - LPN / RN Home Health Nurse
About The Position
Angels of Care Home Health is seeking compassionate and dedicated Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role focuses on delivering patient-centered care according to physician orders and active plans of care, ensuring positive patient and family outcomes through ongoing assessments, medication administration, treatment, care coordination, family education, and preventative initiatives. The company emphasizes a supportive culture founded on love and advocacy for both employees and patients.
Requirements
- Active RN or LPN/LVN license
- Ability to provide care in a client home setting

Responsibilities
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
- Conduct on-going patient care and assessments.
- Administration of prescribed medication, treatments, and therapies.
- Coordination of care.
- Educate family members on patient clinical care to enhance positive outcomes.
- Preventative initiatives to protect quality of care for patient.
