About The Position

Angels of Care Home Health is seeking compassionate and dedicated Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents in their homes. This role is crucial in making a significant difference in the lives of patients and their families. The position focuses on delivering patient-centered care according to physician-prescribed plans, conducting assessments, administering medications and treatments, coordinating care, and educating family members to ensure positive outcomes and protect the quality of care through preventative initiatives.

Requirements

• Active RN or LPN/LVN license (New Grads Welcome, training provided!)
• Provide care in a client home setting
• Ability to make a positive and lasting impression!

Responsibilities

• Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel.
• Conduct on-going patient care and assessments.
• Administration of prescribed medication, treatments, and therapies.
• Coordination of care.
• Educate family members on patient clinical care to enhance positive outcomes.
• Preventative initiatives to protect quality of care for patient.
