About The Position

This position is for a Home Health Nurse to work weekends (Saturday & Sunday) at the brand new Palace at Weston, an Independent, Assisted Living & Memory Care Community. The primary focus of this role is to coordinate patient care and assist with Medicare issues. The nurse will review admission documentation, ensure field staff adhere to agency policies, review and implement patient care plans, maintain daily communication with field staff, review physician's orders, and verify the proper delivery of scheduled and emergent services.

Requirements

• RN License required.
• Must be available to work on weekends, holidays and in emergency situations.
• Must be able to work under pressure and multi-task.
• Self-motivated with a strong work ethic and the ability to work independently AND as a team member.
• Demonstrate excellent verbal communications, interpersonal skills and empathetic listening.

Nice To Haves

• Home Health experience of knowledge of Clinical Terminology preferred.
• Responsible, always on time, resourceful, very accurate and detail-oriented.
• Flexible as to work schedule.

Responsibilities

• Coordinate the care of our patients and assist with any Medicare issues.
• Review all admission documentation in order to ensure the quality of the services to be provided.
• Coordinate with all field staff, ensuring they follow agency’s policies and procedures.
• Review the patient’s plan of care and ensuring that it gets properly implemented, including discussing it with the patients as necessary.
• Maintain daily communication with field staff in order to ensure that all services are provided as per patient’s expectations.
• Review all physicians’ orders obtained by field staff.
• Verify the proper delivery of daily services as scheduled, including unscheduled emergent visits in assigned community.
