About The Position

The #1 Priority of this position is to coordinate the care of our patients and assist with any Medicare issues. This role involves reviewing admission documentation to ensure service quality, coordinating with field staff to ensure adherence to policies and procedures, and reviewing patient care plans for proper implementation. Daily communication with field staff is essential to meet patient expectations. The role also includes reviewing physician's orders and verifying the delivery of scheduled and emergent services.

Requirements

• RN License required.
• Home Health experience or knowledge of Clinical Terminology preferred.

Responsibilities

• Coordinate the care of patients and assist with Medicare issues.
• Review all admission documentation to ensure the quality of services.
• Coordinate with all field staff, ensuring they follow agency policies and procedures.
• Review the patient’s plan of care and ensure it is properly implemented, discussing it with patients as necessary.
• Maintain daily communication with field staff to ensure services are provided as per patient expectations.
• Review all physicians’ orders obtained by field staff.
• Verify the proper delivery of daily services as scheduled, including unscheduled emergent visits in the assigned community.
