Home Health Nurse- LPN/ RN
About The Position
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. This role involves providing skilled nursing care such as trach/vent care, medication administration, and catheter care, monitoring and documenting patient condition and vital signs, assisting with daily living activities (ADLs), communicating with families and care teams, and maintaining safe, high-quality, patient-centered care.
Requirements
- Active RN or LPN license in the state of Michigan
- Strong communication and critical thinking skills
Nice To Haves
- Home care or private duty experience preferred
- Trach/vent experience a plus (training provided)
Responsibilities
