Home Health Nurse (LPN/RN)
About The Position
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. There are high-acuity pediatric openings in West Detroit and Westland. Specific cases include a 6-month-old baby boy with interstitial lung disease requiring Trach, Vent, and NG-tube care; a 1-year-old baby boy who is Trach, Vent, and G-tube dependent; a 2-year-old girl requiring pediatric trach/vent experience; a 4-year-old boy with Trach, Vent, and G-tube management needs; and a 1-year-old girl with Trisomy 21 who is Trach and G-tube dependent and on a ventilator 24/7. The job involves providing skilled nursing care, monitoring patients, assisting with daily living activities, communicating with families and care teams, and maintaining safe, high-quality, patient-centered care.
Requirements
- Active RN or LPN license in the state of Michigan
- Strong communication and critical thinking skills
Nice To Haves
- Home care or private duty experience preferred
- Trach/vent experience a plus (training provided)
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Assist with daily living activities (ADLs)
- Communicate with families and care teams
