About The Position

This position is responsible for assessing patient and family needs, developing care plans in collaboration with the healthcare team, and providing social work support. The role involves participating in multidisciplinary team conferences, maintaining patient records, and establishing goals according to the patient's plan of care. The MSW will monitor patient progress, act as a key resource in complex patient situations, and contribute to cost containment measures. Additionally, the role requires effective communication, representation of the agency, and a commitment to continuous education and professional growth.

Requirements

  • LMSW – Certified Clinical Social Worker Required
  • Master's degree

Responsibilities

  • Assesses patient/family needs in a timely manner upon receiving patient admission, recognizing the patient’s ability, support systems capability, and the patient’s expressed wishes.
  • Assesses patient’s environment and financial needs and makes appropriate referrals to identified community resources.
  • Develops a plan of care in collaboration with physician’s team, patient, and family to meet identified needs and maximize positive outcomes in a timely manner.
  • Actively participates in multidisciplinary team conferences and documents outcomes as needed, assisting the team in understanding the social and emotional factors related to the patient's health problems.
  • Maintains documentation in patients record per internal, regulatory, and professional standards.
  • Establishes short and long-term goals and completes them according to the patients plan of care.
  • Maintains visit productivity to meet patient needs and team responsibility.
  • Monitors, observes, and evaluates changes and progress in patients’ condition and environment and reports changes, progress, or lack of progress to physician and/or nurse case manager.
  • Involves patient and family/significant other in the plan of care while revising and documenting strategies/intervention to accommodate changing needs and/or barriers.
  • Acts as key source in patient situations such as: ineffective patient/family coping and decision-making, advance directives, long-term or assisted living placement, substance abuse, abuse/neglect, and/or bereavement.
  • Demonstrates teambuilding skills.
  • Serves as role model/resource for other team members.
  • Assumes responsibility and accountability for individual social work judgments and actions.
  • Identifies strengths/weaknesses in clinical practice and makes necessary adjustments.
  • Participates in cost containment measures including utilizing supplies efficiently, accurately recording all charges, executing efficient patient flow, utilizing work time productivity, and identifying actual potential problems.
  • Communicates with UR coordinator for any authorizations required per insurance requirements.
  • Maintains strict confidentiality of all Carroll Hospice/CHG system matters including patient, physician, Associate, and financial information.
  • Appropriately represents the agency at meetings both inside and outside the facility.
  • Effectively utilizes proper channels of communication with colleagues, physicians, patients, families, visitors, and outside contacts to ensure clear, concise flow of patient information.
  • Assumes responsibility for continuous education and licensure requirements through participation in educational programs, current literature, in-services, meetings, workshops, etc.
  • Assumes responsibility for professional growth by keeping abreast of changing hospice/home care standards and changing trends in the social work profession and attends and participates in mandatory staff meetings and in-services according to departmental standards.
