About The Position

The Licensed Vocational Nurse (LVN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). The LVN follows the plan of care established by the RN and physician, ensuring high-quality and compassionate care. This role involves direct patient care, documentation, and communication with the interdisciplinary team.

Requirements

  • Graduate from an accredited vocational nursing program.
  • Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure Compact (NLC) member state.
  • Current driver’s license.
  • Minimum one year of clinical experience.
  • Strong clinical skills, effective communication, and ability to work independently.

Responsibilities

  • Provide skilled nursing care as outlined in the plan of care.
  • Monitor patient condition and report changes to RN and physician.
  • Administer medications and treatments as prescribed.
  • Educate patients and caregivers on disease management and care techniques.
  • Document all care provided accurately and timely.
  • Collaborate with interdisciplinary team to ensure continuity of care.
  • Maintain compliance with agency policies, procedures, and regulatory requirements.
  • Participate in in-service training and quality improvement activities.
  • Promote infection control and safety practices during patient care.
