Transition Home Healthcare is an elite home health agency serving Houston, TX and surrounding areas since 2010. LVN or LPN (Home Health) Shifts Available: PRN & Full Time Service Areas for Coverage: Northwest (Spring/Cypress, Tomball, Willowbrook, Inwood, Heights, Jersey Village) Northeast (Crosby, Atascocita, Kingwood, Humble) Southwest (59-288)

Requirements

Valid and current LVN or LPN license in the state of Texas or compact state.
Minimum of one (1) year of experience in a home health setting.
Knowledge of home health regulations and general healthcare compliance.
Excellent communication and people skills to effectively interact with patients, families, and healthcare team members.
Compassionate and empathetic demeanor with a commitment to providing patient-centered care.

Nice To Haves

Bilingual (Fluent in Spanish) encouraged to apply
Experience with HCHB EMR. (preferred but not required)

Responsibilities

Collaborate with the home healthcare team to develop and implement individualized care plans.
Complete home care nursing visits.
Prepare timely, complete, and accurate clinical notes.
Communicate with healthcare providers and other care team members.
Educate patients and their families about health conditions, treatment plans, and self-care techniques.
Adhere to all agency policies, procedures, and regulatory requirements related to home health care.
