About The Position
Puget Sound Home Health is hiring a full-time LPN to join their Pierce County Home Health team. This role offers the opportunity to provide one-on-one care to patients in their homes, with manageable caseloads and flexible scheduling. Requirements
- Current Washington State LPN license, or license in place before your first day
- Current AHA BLS certification
- Strong critical-thinking skills and ability to work independently
- Valid driver's license, insured vehicle, and reliable transportation
Nice To Haves
- Home Health experience preferred, but not required
Responsibilities
- Provide one-on-one skilled nursing care, including wound care, catheter care, medication administration, and symptom management
- Observe and assess patients and communicate changes in condition to the appropriate clinical team members
- Educate and support patients, families, and caregivers
- Collaborate with RNs, therapists, providers, and other members of the interdisciplinary team
- Complete accurate and timely clinical documentation
