About The Position

Puget Sound Home Health is hiring a full-time LPN to join their Pierce County Home Health team. This role offers one-on-one care with patients in their homes, manageable caseloads averaging 4-6 patients per day, and autonomy with support from a collaborative clinical team. Requirements

• Current Washington State LPN license, or license in place before your first day
• Current AHA BLS certification
• Strong critical-thinking skills and ability to work independently
• Valid driver's license, insured vehicle, and reliable transportation

Nice To Haves

• Home Health experience preferred, but not required

Responsibilities

• Provide one-on-one skilled nursing care, including wound care, catheter care, medication administration, and symptom management
• Observe and assess patients and communicate changes in condition to the appropriate clinical team members
• Educate and support patients, families, and caregivers
• Collaborate with RNs, therapists, providers, and other members of the interdisciplinary team
• Complete accurate and timely clinical documentation
