About The Position

The Home Health LPN performs duties under the general medical/nursing supervision and direction of the patient's provider, Home Health Director, and Home Health RN Case Manager. This role involves performing home visits to provide Home Health services according to a written plan of care. The LPN partners and participates with the RN Case Manager in the ongoing review and facilitation of the care plan, patient treatments, including requesting interventions, status updates, and goal setting, as well as addressing family/caregiver needs for assigned patients. The position is responsible for maintaining and facilitating compliance with regulatory processes identified by CMS and JCHH.

Requirements

  • Licensure as a Licensed Practical Nurse in the State of Nebraska.
  • Completes necessary CEU's.
  • Maintains unencumbered license.
  • BLS Certification.
  • Meets all requirements under JCH and JCHH policies for Home Health Nurse - LPN.
  • Maintains a private vehicle and has insurance coverage for own transportation for making home visits.
  • Demonstrates compassion, patience, kindness, empathy, and ability to create a safe and effective environment.
  • Knowledge of health promotion and maintenance.
  • Medical knowledge and understanding of pathophysiology.
  • Proficiency in nursing skills and the nursing process.

Nice To Haves

  • 1-2 years recent nursing experience preferred.
  • Professional or personal experience supporting individuals with disabilities, mental illness, or challenging behaviors highly preferred.
  • 3+ years customer service experience, preferably in human services.
  • Possesses/demonstrates a commitment to customer service.

Responsibilities

  • Acts as a liaison between patient, family/caregiver, RN Case Manager, and physician.
  • Partners with RN Case Manager to plan and facilitate patient care according to individual patient needs.
  • Assists with weekend patient care coverage as designed by the Director.
  • Provides routine care for patients, including head-to-toe assessments, vital signs, and mental health evaluations.
  • Observes and reports patient's health status or changes to the RN Case Manager, consulting physicians, and providers.
  • Assists Physicians and Registered Nurses with ordered care and treatments.
  • Communicates interventions and goals with patients and their family members or caregivers as prescribed by the Plan of Care.
  • Administers oral and injectable medications, refills, and reconciles medications with each visit.
  • Performs standard diagnostic procedures, including phlebotomy/venipuncture or other lab requests that can be completed in the home.
  • Provides wound care and skin management.
  • Monitors, changes, and provides care for catheters or appliances, including PICC dressing changes, LVAD dressing changes, Pleur-evac, Ostomy, ileostomy, Foley, Suprapubic, and flushes IV PICC lines as ordered, and assists with IV infusions.
  • Reviews medical charts within the agency EMR, JCH Cerner, to maintain continuity of care and follow-up.
  • Communicates with families and caregivers.
  • Participates in agency chart auditing as directed.
  • Understands guidelines for compliance and regulatory requirements of CMS and patient payment modes.
  • Completes all required documentation for patients accurately and timely.
  • Has knowledge of state laws governing the scope of nursing practice.
  • Works with area hospitals, nursing homes, and physicians regarding discharge planning as needed.
  • Performs any other duties deemed necessary by the Home Health Director.
