About The Position

Signature Home Health is seeking an exceptional Home Health Licensed Practical Nurse (LPN) to join their vibrant team. They offer both full-time and part-time opportunities. Requirements

• Licensed Practical Nurse (LPN) licensure in Oregon
• 1+ Year of Nursing Experience
• Ability to shine solo or rock it out as part of a dynamic team
• Balancing clinical excellence with an abundance of empathy and zest for life
• Hold a current BLS certification from AHA
• Cruise around town with your own reliable vehicle

Responsibilities

• Provides direct patient care.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Collects assessment data and performs observation to report to and collaborate with RN(s) and physician(s).
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines to the registered nurse and/or Director of Clinical Services.
• Uses equipment and supplies effectively and efficiently.
• Participates in personal and professional growth and development.
