About The Position

You will provide personalized, one-on-one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love. You will report to the Clinical Manager.

Requirements

• Licensed Practical Nurse, licensed in the state of North Carolina, without restrictions
• 2+ Years of Experience as a LPN/LVN in a clinical setting, ideally home health or hospice
• A valid driver's license, auto insurance, and reliable transportation
• Current CPR certification

Responsibilities

• Provide skilled nursing interventions in the treatment of the patient's illness, rehabilitative needs and preventative care, under the direction of a Registered Nurse
• Confer with the RN Supervisor regarding needed changes in the Plan of Treatment
• Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team regarding patient/family needs and status changes
• Participate in care coordination activities and discharge planning
• Prepare appropriate medical documentation and visit or shift reports, updates and summaries of patient records, and confer with other health care disciplines in providing optimum patient care
