Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love.
Requirements
- Licensed Practical Nurse, licensed in the state of North Carolina, without restrictions
- 2+ Years of Experience as a LPN/LVN in a clinical setting, ideally home health or hospice
- A valid driver's license, auto insurance, and reliable transportation
- Current CPR certification
Responsibilities
- Provide skilled nursing interventions in the treatment of the patient's illness, rehabilitative needs and preventative care, under the direction of a Registered Nurse
- Confer with the RN Supervisor regarding needed changes in the Plan of Treatment
- Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team regarding patient/family needs and status changes
- Participate in care coordination activities and discharge planning
- Prepare appropriate medical documentation and visit or shift reports, updates and summaries of patient records, and confer with other health care disciplines in providing optimum patient care
