Requirements

  • Graduate of an accredited school of nursing.
  • Current, unrestricted LPN license in the state of Massachusetts.
  • Minimum of one year of nursing experience in a clinical setting.
  • Strong organizational skills and attention to detail.
  • Excellent communication and teamwork abilities.
  • Comfortable working in a fast-paced office environment while managing multiple priorities.
  • Ability and willingness to travel within the branch coverage area if needed.
  • Valid driver's license, reliable transportation, and current auto insurance in accordance with state requirements.
  • Ability to occasionally lift, push, pull, and support up to 50 pounds, including assisting with equipment or patient-related activities when necessary.

Responsibilities

  • Support the coordination of patient care services and clinical operations.
  • Review and clear HCHB coordination notes and process administrative task workflows accurately and timely.
  • Obtain physician orders, including verbal, written, and faxed orders, and enter them into the appropriate system for review and approval.
  • Review, update, and maintain the At-Risk Registry.
  • Assist with patient-related coordination activities, including scheduling follow-up appointments, obtaining pre-authorizations, providing information to case managers, and addressing patient questions or concerns.
  • Perform chart audits in accordance with company policies and quality standards.
  • Verify receipt of Medicare notices of non-coverage when required.
  • Maintain patient and staff confidentiality in accordance with HIPAA regulations.
  • Promote Elara Caring's mission, values, and commitment to exceptional patient care.
  • Collaborate closely with clinicians, leadership, physicians, and office staff to ensure smooth care transitions and communication.
  • Perform field visits or direct patient care activities as needed.
  • Support additional projects and initiatives as assigned.
