About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, the Home Health Licensed Practical Nurse provides personalized, one-on-one care to help patients regain independence in their homes. The nurse works with physicians and clinicians to manage care plans that support recovery. The LPN provides skilled nursing care in patients’ homes under the direction of an RN, collaborates with patients, families, and the healthcare team, and contributes to care coordination and discharge planning.

Requirements

• Current nursing license in the practicing state.
• Valid driver’s license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting.

Nice To Haves

• Nursing experience in a Home Health or Hospice setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
• Implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Educate patients and families on disease management, treatment options, and self-care techniques.
• Maintain accurate records of patient visits, interventions, and outcomes.
• Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
• Participate in care coordination activities and discharge planning.
