About The Position

As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. You work with physicians and clinicians to manage care plans that support recovery and help patients regain independence. The role includes patient and family education, care coordination, and discharge planning. This is a patient-facing position subject to Humana’s TB screening program and driver safety requirements. Requirements

• Current nursing license in the practicing state.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification.
• Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities

• Provide skilled nursing care to patients in their homes under the direction of an RN.
