Home Health Licensed Practical Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one care to help patients regain independence at home. You work with physicians and clinicians to manage care plans that support recovery. The role provides skilled nursing care in patients’ homes under the direction of a registered nurse, and includes patient and family education, care documentation, communication with the care team, care coordination, and discharge planning. This is a patient-facing position covered by Humana’s tuberculosis screening program and driver safety program. The position is listed as 40 scheduled hours per week.
Requirements
- Current nursing license in the practicing state.
- Valid driver’s license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.

Responsibilities
- Provide skilled nursing care to patients in their homes under the direction of an RN.
- Implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Educate patients and families about disease management, treatment options, and self-care techniques.
- Maintain accurate records of patient visits, interventions, and outcomes.
- Promptly communicate with patients, physicians, clinical managers, and other healthcare team members about patient or family needs and changes in status.
- Participate in care coordination activities and discharge planning.
