Home Health Licensed Practical Nurse
About The Position
As a CenterWell Home Health nurse reporting to the Clinical Manager, you provide personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. You work with physicians and clinicians to manage individualized care plans that support recovery and help patients regain independence. The role includes educating patients and families about disease management, treatment options, and self-care; documenting visits, interventions, and outcomes accurately; and promptly communicating patient or family needs and changes in status to patients, physicians, clinical managers, and other healthcare team members. You also participate in care coordination and discharge planning. This is a patient-facing role subject to Humana's TB screening program and driver safety requirements. The position is listed at 40 scheduled hours per week.
Requirements
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting.
- Maintain vehicle insurance at the residing-state minimum required limits or the stated alternative limits, whichever is higher.
- Complete TB screening if selected for the role.
